How do I?

Register as a patient

You can now register with us submitting an online registration form or register in the practice by completing a paper form.
Please send a copy of your ID and proof of address.
Please make sure you put your NHS number on the form. If you are new to the NHS please put the date you entered the country.
Please note that we do not register children without the parents of the child registered with us.
To avoid delays in your registration, please ensure you complete all fields on the registration form.

Make an appointment

If you are housebound (i.e., unable to get to the surgery) and feel that you may need to see a doctor, please contact the surgery on 02085943667 before 10.30 am. The receptionist will discuss your concerns with the doctor and will update you of the outcome.

E-consult is an online consultation enables you to contact a GP or other health professional over the internet. It saves you waiting for an appointment or going to the GP surgery. You can tell your GP about your health using a smartphone, tablet or computer. The type of online consultation you have depends on your needs and the nature of your request – face-to-face, telephone, email or SMS.


Use E-Consult to ask your GP surgery about your health symptoms, conditions or treatment. You can even request things like sick notes and GP letters.

* Fill out a quick form.
Your practice responds by phone, SMS or email, usually by the end of the next working day.

This allows patients to submit their symptoms or requests to their own GP electronically, and offers around the clock NHS self-help information, signposting to services, and a symptom checker.
Designed to enhance patient access, improve practice efficiencies and signpost patients to the right place at the right time for their care.

Order Repeat prescriptions

Repeat prescriptions can be ordered in the following ways:
 Through pharmacy
 Log in to online services and order it online
 In person at the practice
 Via post
We do not take prescription request over the phone.

Obtain results

When you attend for a test of any kind you will be told how long you should expect to wait for the results.
Our reception staffs are not qualified to comment on results therefore it is your responsibility to check them and make any necessary follow-up appointment with the doctor.
Please note that we do have a strict policy regarding confidentiality and data protection. In this respect we will only give out results to the person they relate to unless that person has given prior permission for their release or if they are not capable of understanding them.
How to request/view your test results?
You can submit an e-consult request via administration help to ask for your test results. If you are not already registered for GP online services please let a member of the practice team know you would like to use this service or follow the link to start the registration process.
