Registering with the Practice

You are welcome to register as a patient with this practice, if you live within our catchment area, which is defined in the map lower down on this page.

All new patients will be asked to complete a medical questionnaire and registration form – Register here

If you are aged between 40 and 74 and have not had an NHS health check in the past 5 years, please speak to one of the patient care co-ordinators to arrange this.

Practice Area

The practice serves an area between the rivers Thames and Cherwell and the eastern bypass. It also serves Littlemore, Sandford, Blackbird Leys, Garsington and Horspath.

How to Register

Registering with our practice is a simple process.  Visit us during our opening hours* and our patient care co-ordinators will provide you with forms to complete.  Bringing ID helps us ensure we get the spelling of your name and date of birth correct.

We aim to process all registrations within two working days, after which you can use all of our services.  However, if you need to speak to a doctor or other health professional and the problem cannot wait for two days, please explain this to our patient care co-ordinators.

Once you have registered we recommend that you sign up for online services which will allow you to book and cancel doctors’ appointments and request repeat medications online 24 hours a day.
