You can register with us online or by handing a completed registration form to our reception. Please click on the link below to register with us online.
Alternatively, you can register by filling out a registration form which you can download here or collect from our reception. Please fill it out completely and give it to the reception on Monday, Tuesday or Thursday between 12~3pm. Incomplete registration forms will be turned away.
A photo ID, usually a passport or a driving licence, is not compulsory but bringing one and/or your NHS number with the completed form will help make the registration process smoother.
As part of your registration process, you will be required to have a routine new patient health check to complete your registration process. An appointment will be offered with our Health Care Assistant during your registration.
Please note – we only register patients in our catchment area – primarily the Balsall Heath area, B12. You can check whether you are in our catchment area by seeing whether we are your closest GP on the NHS website below.
We are pleased to welcome new patients however they need to be living within the catchment area. Please make sure you live within the blue area of the map before registering.
