Patient Online Services

If you wish to be able to view your ‘Detailed Coded Records’ such as test results and consultations please complete the attached form and bring it to the practice with some photo identification and proof of address.

We will then review your application and grant access if appropriate. Please note this may take the practice up to 14 days to action and in some circumstances may take longer.

If you wish to give a third party access to online services on your behalfs such as a family member or carer, please complete the form below. You may also use this form if a patient lacks capacity and you have been granted a power of attorney to manage the patient’s medical affairs.

  • View your summary care record
  • Repeat prescription

all via the internet from home, work or on the move, and in your own time, day or night.

You can find out more about Emis Access and how to register for this service by calling into the surgery with two forms of identification (one item from each of the following lists):

  • Personal ID: Current passport, photocard driving licence, birth certificate etc.

AND

  • Address ID: Utility bill, bank statement or council tax etc (with your name, address and dated within the last 6 months).

You will then be given your confidential login information.

All information that is sent to the Surgery via EMIS Access is secure. Your personal details are encrypted and protected using the highest standard internet security, so it cannot be intercepted. Only you and the surgery are able to see this information.
